ISSUANCE OF CERTIFICATE OF NO PENDING ADMINISTRATIVE CASE (WALK-IN/BY MAIL)

Schedule of Availability of Service:

  • Monday – Friday
  • 8:00 a.m. – 5:00 p.m. without noon break

 

Who May Avail of the Service:

  • Government officials and employees and other authorized individual/officer

 

What are the Requirements:

  1. Accomplished Customer Information Sheet (CIS) and Request for Certificate of No Pending Administrative Case Form;
  2. Certified photocopy of valid ID of the requesting party. If request is through a representative, authorization letter and valid ID of the representative.
  3. Proof of payment of certification fee (except if the purpose is for compulsory retirement or for candidate of Honor Awards Program)
